You can find these settings under the "Edit blog" section.
- Click on the "user" icon on the top menu, it's that little "human" icon second from right.
- Select "Edit blog"
Fill out and change the settings of your blog into your liking, when you're done, simply click "Save".
That's it, your blog's title and description is now changed.
